Medium size monsters already die when wounded once, which doesn't apply to players. And Great Size monsters usually are, well, MONSTERS: trolls, dragons, the sons of Shelob, and the Werewolf of Mirkwood. I can hardly imagine such beasts suffering wound penalties.

There's no symmetry between monsters and characters, so fairness is a debatable thing.

This was actually first proposed on the old forum, and I have been using it ever since as it makes sense ( there should be a penalty for being Wounded), but I take this further:

A character who is Wounded OR Weary ignores rolls of 1,2 & 3 on success die.
A character who is Wounded AND Weary also ignores 1,2 &3 on the Feat die.

This makes heavy armour more important.

This makes the game's 'death spiral' a lot more lethal (since Weary characters are more likely to get Wounded), so I'd think very carefully before doing it.

Wounds already suck a lot in the long-term, making the recovery of damage a great deal harder and making the character a lot more fragile until its all healed...they just aren't debilitating in combat, which seems thematically sound as well as pretty fun...so this kind of rule doesn't really seem necessary.

That is my other objection. This turns TOR into a character grinder.

"Damnit, that's the 3rd character I lost this month!"

I do wounded counts as weary only when the hero who is wounded roll his next Sauron, this is a nice compromise to help minimize the death spiral effect. Often the Wounded dont also get wounded unless its a long battle. They often can get Treated before that happens. Treated Wounds are not subject to this HR.
