What does "otherwise harmed during play" mean? Are they talking about Endurance loss? For example, Frodo is Sam's Fellowship Focus. At the end of a session, Frodo is not wounded, but he did suffer Endurance loss. Does that qualify as "otherwise harmed" and thus Sam cannot recover one Hope Point?

I'd take "harmed" to mean Wounded, Poisoned, Spent, or any other serious condition (but not Weary or Miserable). But it's a judgement call: the rules don't really specify exactly which conditions count.

I agree with trystero: I would read 'otherwise harmed' as poisoned etc

Poisoned adds a wounded state as well, so the "Fellowship focus wounded" would definitely apply.
