I like the idea that the players do something stupid or cause some accident when they roll an EoS.
My idea is for all the rolls trying to remedy the situation still counts as failures toward Tolerance, but the accumulation of successes toward the overall result of the encounter doesn't count until the situation is under control again.
I find that players almost never fail an encounter. One of our players have the Noble Armour, so Tolerance is almost always 6 or more. It is not hard to score 7 successes in 6 tries, when they have 3-4 ranks in the relevant skills.

Eye of Sauron should cause Hazards in all 3 phases of TOR. Preliminary rolls on all 3 phases started the trend, Hazards should follow!
