I started trying to build something like that, but I focused on the CSS first, trying to make it look like the official sheet. Here it is in its partially completed state: http://codeasart.com/torsheet/

Feel free to lift the CSS and other resources if you want.

I will see if I can apply the CSS into my profiler version. I am a novice when it comes to writing in PHP script.
