Does anyone have a form that players can use to record stuff about their company, like company songs?

For our List of Songs, I merely typed them all out in a single document. I didn't make any kind of form for it, I'm afraid.

When Rivendell first came out our Loremaster wrote the songs on a nice piece of parchment with a pen in ink. Very flavorful. Of course you could easily do this in a word document, just pick a nice script font.
