I understand the logic of saying that vicious adversaries would use No Quarter, or that "chieftan's are scarier than non-chieftans" and thus would use it, but then why don't all vicious and chieftain adversaries have the ability?

Clearly logic alone isn't sufficient to justify its presence.

I see it as those who have it are simply more efficient at killing and/or are consciously or innately more intelligent in terms of self-preservation ('dead people don't stab me in the back when I'm fighting good guy #2' or 'that's one less troublemaker when we're marching the slave train back to our territory').

Vicious adversaries that do not have it can be explained away as being dumb,sloppy, or careless, or have inflated opinions on their battle prowess ('I stuck him and he went down, obviously I must have killed him, who's next! kill! kill! kill!')
Don't start arguments over who has a better grasp of hiking and boating or someone might just bring down the banhammer.

So Hill Troll Chieftains are more intelligent than Nazgul?

I think it's problematic to try to associate No Quarter with specific traits, like viciousness or intelligence. It's just never going to map cleanly.

Inflated ego can surely be better applied to a Troll Chieftain than to a Nazgul

I have no problem with "No Quarter", there are enemies that must be feared and PCs can and shall die from time to time.

I've never really seen the Nazgul as throat cutters. Even though could easily be as the Black Breath could easily render a PC unconscious.
