Last night while playing through Crossing of Celduin from Tales, my players company competed in the grand melee. At the end, three players remained having barely defeated all other opponents. They opted to fight it out to see who was the grand champion. We ran into the fact that there were no rules for this as far as choosing stances. What we did was have everyone role for initiative. Then in order, they chose who they wanted to fight, the defender chose the stance. It worked out really well.

In my houseruled game, I use something like this:
Open Stance: defence=9+Parry
Aggressive Stance: defence=6+Parry; roll 1 extra d6 when attacking
Defensive Stance: defence=12+Parry; roll 1 less d6 when attacking
Open Stance: defence=9+Parry
Aggressive Stance: defence=6+Parry; roll 1 extra d6 when attacking
Defensive Stance: defence=12+Parry; roll 1 less d6 when attacking

I hope it never comes to Fellowship members deliberately fighting outside of the Bout of Madness, so far, so good, but we had recently PCs combat sparring and we needed to come up with something.
In the middle of Zed’s TotM, between the Festival trials, to pass the time (Barding was teasing there is no archery contest), two PCs decided to fight “a three round” sporting match to see who is “a better” spearman (old teacher vs old pupil). They declared no intention of drawing blood (it would automatically stop the fight), so the pikes were wrapped in the leather rags, Edge changed to Eye Rune only (Gandalf stayed just as the direct success without piercing), Injury raised by two levels and all damage assumed healing x2 normal after the fight. Both were fighting (armored with shields, helmets and leather shirts) in the same defensive stance since we ruled both will assume the higher TN stance from the initially (secretly) chosen. Pupil won the game receiving less damage.

I've never really seen the need for it.
That being said the simplest solution might be to treat both combatants as in open stance.

I am thinking of a badly tainted/corrupted PC (maybe 4th level madness) that would still be role-playable...
But if its an uncontrolled bout of madness then I see now that normal rules apply as the PC becomes NPC for a while...
