1st level - ordinary - either beat the difficulty or roll a gandalf, no sixes shown on the d6's.
2nd level - Great - as ordinary, but one of the d6's shows a 6
3rd level - Extraordinary - as ordinary, but 2 or more 6's show a 6.
Further, they are lost at the end of whatever they were generated for.
Oh, and while you probably aren't there yet, RAW, you never roll more than 6d6.
Note that Inspiration from songs (Rivendell) adds 2d to one task (again, still capped at 6d).

Everytime you roll, there's a maximum of 3 successes per roll. Never more than that.
So you can do 3 levels of damage in a single hit, or count three successes to an Encounter with a single skill,... but never more than three.

For clarity's sake, I'm pretty sure aramis means that there are a maximum of three Bonus Dice for any phase that involves Preparation.
For example, in a single combat, each hero can earn a maximum of three Bonus Dice. Each Bonus Die can be used only once, and a maximum of one Bonus Die can be used per roll. Note that since Bonus Dice can be shared, a single hero could use five Bonus Dice in five different rolls. No matter where the dice come from, you can only roll a maximum of six Success dice in a single roll.
The same applies to a Journey or an Encounter; each hero can have 0-3 Bonus Dice due to Preparation and only one Bonus Die can be used per test.
