We've had probably one Revelation Episode every other session, other than a stretch where I tried out zedturtle's variant (where it often got close, but nobody quite triggered one). I've used many of those listed in the book, and I tend to agree with what's been said: that you want to add to atmosphere and the mood, but not just devastate the party.
One thing to keep in mind is that - much like a Bout of Madness - the Revelation Episode doesn't have to be done right away. Sometimes it's better to let it wait, and give the session tension, knowing that at any time the hammer might fall...

So far I have only had one Revelation. They were travelling under the Misty Mountains trying to find an old forgotten Dwarven City/Hold. The reason was that one of the Dwarves in the party had a cursed magic item and his research led him to some old books and maps that eluded to an old spell that could remove the curse, as well as a 'great treasure' held there and 'great enemy' that had killed and driven away the population many centuries previously.
I had planned out a map of the route that would lead them there, with some encounters and ideas for Hazards, should any arise. The Revelation happened as they were skirting around a huge chasm, the bridge across having long fallen, and some parts of the path were very narrow, requiring Athletics tests. And one of these tests ended up triggering the Revelation.
I had a creature attack the group, the only part of the monster they could see were the large tentacle-like arms that were attacking them, I based it roughly on the Thing in the Pit from the Tales of Wilderland book. The fight wasn't planned so the Revelation was that this slumbering 'thing' had been awakened and was beyond the skills of the party to defeat. The only chance they had was to reach the far side of the chasm, whilst defending themselves. One of the party was almost killed as she had been grabbed by one of the tentacles and was almost dragged down into the depths. It was a really fun encounter as they slowly realised the creature couldn't be defeated and they had to flee.
I do like the mechanic and it's always great to see the looks of worry as the Eye Awareness grows.
