I for one would be very hesitant before you did anything that might nerf a PC. Some players will roll with it, but most will resent it, even if they do not admit it at the time. But if you really want to go down this track, then make sure that whatever injury or wound they suffer, each gains a corresponding benefit which just slightly but perceptibly outweighs the detriment inflicted: e.g. Arvi the Dwarf's lungs are never quite the same after his time in the dank fungi filled dungeon, so in future he has to raise all Athletics and Travel TN's by 2 unless it is Summer, however, from his deep reflections during his time of incarceration, he conceived all sorts of beautiful objects which he crafted with only his imagination and imbued them with the very fibre of his being, so in future he adds his Wisdom bonus to Craft and Lore tests associated with making, shaping or understanding items.
