Revised Core - Page 160: The Loremaster determines what will be the consequences of the Hazard episode, should a companion fail the challenge posed by it.

However, skywalker is correct, if nobody steps up, the Loremaster is encouraged to make everybody take the hit. In point of fact, the Loremaster can decide everybody takes a worse hit for their general cowardice in everyone failing (or refusing) to spend a Hope and at least try to mitigate the Hazard by taking it upon themselves.
