Lie detection is certainly part of what players can do with Insight, but I see Insight as part of the interaction, thats why when it fails, you'll use up a tolerance point.
So with Insight you are better at detecting what subjects the target are avoiding. If your insight is good, then you'll know how to set your target up to reveal a lie.
I agree that a straight up was this true or false is random at best, but as part of an interaction, then it is not only possible to set a target up, but very likely that the target will fail confronted by a skilled interrogator. At the very least the target will stop answering the questions at which point you'll know that he is lying or hiding the truth.
