I didn't mean that nicknames should form surnames - I was trying to give him a compromise between the two extremes so he could allow his player some leeway ;)

But when you look at it, aside from the Hobbits who have surnames (eg. Took, Baggins, Gamgee, Brandybuck, etc.) and the people of Bree-land ( eg. Barliman Butterbur, Bill Ferny)...Not sure if it applies to all of Eriador; most culture and people in Middle-Earth seems to only use a Given Name with sometimes a Nickname or a Patronymic.
