Document Backgrounds

Quick question: Where are the TOR backgrounds for fanmade documents? They seem to be hiding from my google searches.

Also, I'd like to post an adventure I created, but I used some enemies from the Lake Town supplement. Is it ok if I put those statistics in there? Or should I just refer a page number? And are pictures from there kosher?
