That describes what happens when a player-hero experiences a bout of madness; he might attack another player-hero. But during a bout of madness the character becomes a Loremaster character, so the normal rules for combat may still be followed (more or less).
The main issue with true player-hero–versus–player-hero combat is that of choosing combat stances. I suggest either allowing whoever has the initiative to choose stances, or using the most aggressive stance selected as the stance used.
On the other hand, unless the characters are really enemies, you can just treat the combat as an opposed action. Whoever loses has his endurance reduced an amount decided by the Loremaster. Combat caused by violent argument among people who are usually friends is unlikely to result in wounds or death.
